I want to come in the fall due to a planned surgery in the winter.

When do the skeeters start to really decrease such that they are manageable.

After the first cold front but it varies in different areas.
My first trip last season was a loop from EC, Picnic Key to Pavilion Key on the last week of November, the first night at Picnic the no-see-ums were unbearable it was warm and zero wind, I watched a spectacular sunset from the inside of my tent. The following night at Pavilion it was still warm but a steady breeze kept all bugs at bay and I enjoyed a bug free evening.

Some people try to camp at beach sites at the beginning and end of season and ground sites when in season, but keep in mind that the Everglades is unpredictable so you have to be prepared.
